What Documents Are Required for Preschool Admission? A Parent’s Checklist

Admission season sneaks up on parents. One week you’re thinking “we have time”, the next you’re searching for a birth certificate at 11 pm because the form closes tomorrow.

This guide gives you the full list of documents required for preschool admission in India, what each one is for, what to do if one is missing, and a printable checklist — so you can put one folder together this week and stop worrying.

Requirements vary by school, city and state. Always confirm the final list with the preschool you’re applying to.

When to start gathering documents

Most preschools admit through the year, but the main season for the April academic year runs roughly November to February, and popular centres fill early. Start gathering documents at least four to six weeks before you plan to apply — some (like a corrected birth certificate) can take weeks.

The core documents (almost every preschool asks for these)

1. Child’s birth certificate

Why: proof of date of birth, used to confirm the right class for your child’s age.

  • Must be issued by the municipal corporation / local registrar (not just the hospital discharge summary).
  • Check that the child’s name, date of birth and parents’ names are spelt correctly — mismatches cause problems for years.
  • For children born on or after 1 October 2023, the Registration of Births and Deaths (Amendment) Act, 2023 made the birth certificate the key single document of date of birth for purposes including school admission.

No name on the birth certificate yet? Many families register a birth before naming the baby. Apply to your local registrar to add the name — do this early.

2. Passport-size photographs of the child

Usually 4–6 recent colour photos, plain background. Keep a soft copy on your phone too.

3. Photographs of parents / guardians

Often needed for ID cards and the pick-up register.

4. Parents’ ID proof

Aadhaar card, PAN card, passport, voter ID or driving licence of both parents.

5. Address proof

Any one, usually: Aadhaar, passport, electricity / water / gas bill, registered rent agreement, or a bank passbook with address.

  • Renting? A registered rent agreement plus a recent utility bill usually works.
  • Some schools use distance from home to school in admission criteria (especially in formal school nursery admissions), so make sure your proof shows your current address.

6. Immunisation / vaccination record

Your child’s vaccination card, showing vaccines given so far. Many preschools ask for this to protect all children. Keep it up to date — your paediatrician can provide a copy.

7. Medical information form

Blood group, allergies (food, medicine), ongoing conditions, regular medicines, and your doctor’s contact. Put allergies in writing — never rely on mentioning it at the gate.

Documents you may need in some cases

Situation

Document

Child’s Aadhaar available

Child’s Aadhaar card (Baal Aadhaar) — often requested but generally not mandatory for school admission; ask the school about alternatives if you don’t have one yet

Switching from another preschool

Transfer certificate (TC) or leaving certificate, and last progress report

Single parent / guardian

Guardianship documents, or relevant legal documents if applicable

Adopted child

Adoption deed / order

Child with special needs

Medical or assessment reports — helps the school plan support

Claiming a sibling or staff concession

Sibling’s ID card / employee proof

RTE / EWS quota (formal schools, some states)

Income certificate, category certificate, as specified by the state

Moving from abroad

Passport, visa / OCI, and records from the previous school

About Aadhaar: many schools ask for it, but in 2018 the Supreme Court ruled that Aadhaar cannot be made mandatory for school admission. If your child’s Aadhaar is still being processed, tell the school — the birth certificate is the main proof of age.

The other forms you'll fill at admission

Beyond documents, most preschools also ask you to complete:

  • Admission / registration form — child and parent details.
  • Authorised pick-up form — names, photos and ID of adults who may collect your child.
  • Emergency contact list — at least two numbers besides parents.
  • Medical consent form — permission for first aid and emergency treatment.
  • Photo / media consent — whether your child’s photo can be used on social media.
  • Transport form — if using school transport.
  • Fee agreement — read the refund and notice-period terms carefully.

Printable preschool admission documents checklist

☐ Birth certificate (original + 2 self-attested copies) ☐ 4–6 passport-size photos of the child ☐ 2 photos of each parent / guardian ☐ Parents’ ID proof (copies) ☐ Address proof (copy) ☐ Child’s Aadhaar (if available) ☐ Vaccination record (copy) ☐ Medical / allergy information (written) ☐ Blood group ☐ Transfer certificate (if switching) ☐ Previous school report (if switching) ☐ Authorised pick-up adults’ photos and ID ☐ Emergency contact numbers ☐ Special category documents (if applicable)

Tips:

  • Make 2–3 self-attested photocopies of each document.
  • Scan everything into one PDF on your phone — many schools now accept online forms.
  • Keep originals in a single folder and take them to the admission meeting for verification.
  • Name every file clearly (e.g. Aarav_BirthCertificate.pdf).

Common mistakes that delay admission

  1. Name mismatches — different spellings on the birth certificate, Aadhaar and form. Fix these early.
  2. Hospital certificate instead of municipal birth certificate.
  3. Outdated address proof after a move.
  4. Missing vaccination record — ask your paediatrician for an updated copy.
  5. Leaving the TC too late when switching preschools — request it as soon as you decide.
  6. Not reading the fee and refund terms before signing.

Beyond documents: check the age criteria

Documents prove eligibility; age decides the class. Typical guidance in India is roughly Playgroup ~2–3 years, Nursery ~3–4, Junior KG ~4–5, Senior KG ~5–6, but cut-off dates vary by state and school. Under NEP 2020, the foundational stage covers ages 3–8, with formal Class 1 entry generally at age six in most states.

Joining mid-year?

If your child is joining after the session has started, the documents are the same — plus a TC if switching. Read mid-year preschool admission: what to look for and mid-term admissions: is it too late to start school?. Once your child starts, how to help your child adjust after the school year has started helps with settling in.

Applying to daycare too? Most daycares ask for the same documents — see our daycare checklist for working parents and daycare vs preschool.
